COVID-19 NOTES AND PRECAUTIONS:
-- All DBTC 2022 Out-of-Town Tours require that participants be fully vaccinated; this includes 2 Pfizer or Moderna vaccinations + booster or 1 (one) J&J vaccination + booster. No pre-tour negative COVID tests will be accepted.
-- You are responsible for supplying your own masks and hand sanitizers.
-- Wear a mask in any of the public spaces within the hotels and/or when using public transportation.
-- If in the 2 weeks prior to tour date you develop COVID-19 (or variant) symptoms, have had close contact with someone who tested positive, or a household member tests positive, you are asked to cancel from the tour.

TO REGISTER: Please mail a $500 deposit check payable to Marilyn Choske, along with these completed forms: DBTC Liability Waiver, Registration/Medical form, and COVID-19 Policy Form to: Marilyn Choske, 3900 Holland St., Wheat Ridge, CO 80033. Balance of $950 is due by April 15, 2022. Applicants will be added to the tour roster on a first-received-forms basis.
NOTE that registration funds are nonrefundable but may be transferable. To cover your tour registration fee in the event of unforeseen circumstances preventing you from participating in the tour, DBTC recommends travel insurance.
